Clinico-biochemical evaluation of phytopreparations using to treat patients with chronic generalized catarrhal gingivitis

Among the actual problems of modern dentistry the periodontal diseases occupy leading positions and although the prescription study there are many moments in diagnostic and treatment remain unclear. Due to the constant increasing patients with inflammatory diseases of periodont in young age, the most urgent problem is detection and treatment of periodontal disease in the early development stages .

For treatment and prophylaxis the peridontal tissues diseases basically used preparations with chemotherapeutic effect,which have number of side effects with negative influence on the human body. That's why there is necessity of alternative therapy, based on a biological needs of the body, such preparations, in our opinion, are phytoestrogens and it using in treatment of periodontal disease. Phytoestrogens - representatives of the class of bioflavonoids, exhibiting estrogenic properties.

The purpose of our investigation is scientific justification of phytopreparations use efficiency in complex theraphy of peridontal diseases , namely chronic generalized catarrhal gingivitis.

The missions of investigation are:

1. To study the clinical aspects of chronic generalized catarrhal gingivitis in young people.

2. To develop the complex etiopathogenic therapy with phytopreparations use in peridontal diseases treatment .

3. To substantiate the favorable influence of phytoestrogens on enzymatic activity of mixed saliva for peridontal diseases.

4. To analyze efficiency of chosen method of treatment and prophylaxis in patients with chronic generalized catarrhal gingivitis.

For the first time in dentistry clinical practice proposed the new effective complex of treatment, which has both general effect on the body and local on the peridontal tissues, and includes the use of phytoestrogens, contained in supplements “Biotrit-C” and “Biodent -3”.

The material was the mixed saliva , taken off 61 patients, divided on 3 groups. The first group consist of 25 patients(10 males and 15 females),whom were carrying out the complex treatment of chronic generalized catarrhal gingivitis; second group – 21 patients (14 males and 7 females),whom were carrying out traditional treatment and third group, which consist of 15 people(7 males and 8 feamales) was control.

The traditional treatment consist of general increasing resistance of the. For the first group,which received complex treatment, additionaly to traditional treatment was prescribed mouthwash by dental elixir “Biodent -3” on the basis of extract of wheat germ, developed by Institute of dentistry of Odessa and scientific and production association "Odessa biotechnology"(mouthwash 3 times a day after meal 1-2 teaspoon to 1/4 glass of water).The course of treatment – 14 days – 2 courses per year. To this group also was prescribed preparation “Biotrit-C”- broad-spectrum biomodulator ,which contains free amino acids, vitamins, microelements, ascorbic acid. This preparation stimulate defenses of the body , activate phagocytosis and humoral immunity, increases stability to the infection, toxic substances and radiation, promotes the lowering cholesterol.(1 tablet 3 times a day, the course of treatment – 20 days,2 times a year). In complex therapy also was used one of the last development of the company “Satelec”, Germany - ultrasonic scaler “P-max”, apparatus for scaling, deep gum cleaning, injection of medicines, supra- and subgingival calculus removal.

By Schiller and Pisarev sample was estimating the prevalence and degree of inflammatory process.

During the clinical examination of parients ,who had conducted complex therapy, with using phyotoestrogens and apparatus P-max observed gum bleeding reducing and elimination of bad breath in 67,5% after 5^th procedure,hyperemia and edema decreased in 43,8% of patients. Disappearance of hyperemia and edema observed after 6-8 procedures in 92,4% of patients.The Schiller and Pisarev sample was negative in 70,36% of patients ,weakly positive – in 17,53 and positive – in 12,11%.Besides, after ultrasonic cleaning in observed group the hygiene of the oral cavity has significant improved.

In patients of second group ,where used traditional therapy , the positive changes in clinical picture after 7-8 procedure still insignificant. Elimination of bad breath and gum bleeding reducing were in 53,4 % of patients.The significant mucousal edema reduction noteced in 62,6%. The Schiller and Pisarev sample was negative in 58,3% of patients ,weakly positive – in 23,2% and positive – in 18,5%. The analysis of capillaries gums resistance allowed to mention the positive dynamics in all patients,which had the complex therapy(main group)

Biochemical analysis have showed decrease of antioxidant activity of saliva in first group (0,038±0,004 mkM/l to 0,035±0,004 mkM/l) by 5 day. In second group AAS remains high(0,038±0,005mkM/l). Approximation to control indexes is noteced by 15-30 day in main group.

In this way the conducted clinical, fuctional investigations and monitoring of mixed saliva enzymatic activity in young pepople, suffering from chronic generalized catarrhal gingivitis allow to make conclusion:

1. Using the complex therapy, which includes preparations “Biotrit-C” and “Biodent -3” and ultrasonic scaler “P-max” leads to decrease of indexes after 6-8 procedure and normalization of mixed saliva enzymatic activity by 5-7.

2. The complex etiopathogenic therapy contributed to increase the efficiency of conducted therapy.In first group normalization of peridontal tissues condition noticed to 6-8 day, in second group - on 8-10^th day .

The complex treatment of peridontal tissues inflammatory diseases leaded to convalescence 19 from 25 patients ,what amount to 74,4%,no changes- 2,7%. The deterioration wasn't obsereved,0%. The patients, which received traditional therapy improvement was in 25,5%.

In this way , the received indexes point to the high effect of conducted complex therapy by phyotoestrogens in treatment of peridontal tissues inflammatory diseases.

The positive results give a reason to recommend the proposed method of treatment in practical health care.
