Use
of innovative technology inhalation of oxygen- xenon mixture at dental implantation
(A literature review)
Abstract. Current problems in dental implantology
is the problem of eliminating the fear of the upcoming surgery, an effective
analgesia, due to soreness, lasting, traumatic ongoing operation. The operation
without pain and fear is the foundation of modern dentistry. Innovative
technologies in anesthesia is the use of oxygen-xenon mixtures.

In the Russian Federation
the use of medical Xenon (KseMed®) was first allowed by order of the Ministry
of Health from 08.10.1999 ¹ 363 "On the resolution of the medical use of
drugs" as a means of inhalation anesthesia [1, 7]. For a long time the use
of dental health Xenon is not found, it prevented the economic aspects and the
lack of specialized medical equipment. Through scientific corporation
"Biology Gazservis" unique device Stuckey was developed in 2006. It
became possible to significantly reduce xenon consumption, making it more
affordable method of xenon treatment on the dental admission. [2] June 23-24,
2011 at the Congress - the center of a special economic zone "Dubna",
ZAO "ATOM-MED Center" together with JSC "Tyumen aerosols"
were presented samples of the first batch of active drugs xenon mixture with
oxygen under "Medksenon" trademark in aerosol, which has increased
its availability.
The first experience of
Medksenona when installing implants held in 2013 specialists of JSC
"ATOM-MED CENTER" and professor of family clinics, IA Shugaylova
"INTELSTOM". It was held a successful one-step installation of 2
implants mandible patient 65 years. Premedication with sedatives and atropine
was not carried out. Inhalation drugs "MEDKSENONA" at a concentration
of Xe: O2 = 70:30 was held for 3 minutes in the dental chair unit for xenon
inhalation (xenon therapy) "MIGA-AMC -1". Consumption on the
inhalation of xenon was 3 liters. Performed articaine
anesthetic infiltration anesthesia in an amount of 1.5 ml by the standard
method. The duration of surgery was 47 minutes. The patient was calm, able to
moderate sedation. Blood pressure, pulse rate and SpO2 remained stable
throughout the operation. After surgery, the patient self-rose from his chair.
According to the patient's body is maintained relaxation, and his head was
clear. Thus, the use of "MEDKSENONA" in dentistry implant and as premedication
- additional analgesia and sedation, to relieve the patient of mental and
emotional stress in the dentist's chair shown to be highly effective [3].
It is no exaggeration to
call the application of the stunning results that actively develops and
implements the chief doctor "AVERON" dental clinic Sitnikov E.V. at a
reception in April 2011: "The use of xenon significantly increases the
effects of other painkillers, analgesic effect is maintained up to 6 - 12 hours
depending on the intensity of pain. For relief of severe pain, only one
three-minute inhalation, with the patient's consciousness is stored. Xenon has
the additional pronounced anti-stress effect. During the long-term implant
operations, I use a combination technique of combining traditional and
inhalation anesthesia with xenon. Patients after surgery feel great, leave the
reception in a good mood. And, very importantly, through this procedure, the
healing process is accelerated and flows smoothly. And most importantly - to
ensure comfortable stay of patients in the clinic and in the future [4].
Results of clinical studies
Sitnikov E.V. demonstrated chronology procedures oxygen-xenon inhalation to
relieve pain after surgery, sinus lift with simultaneous implant placement: paresthesia of the extremities in 50 seconds, nystagmus in
1 minute and 15 seconds, a reduction in pain 1 minute 40 seconds, reducing pain
2 times - 2 minutes 25 seconds, maintaining the level of pain or in the initial
3 minutes 25 seconds. Inhalation Duration reached 5 minutes 05 seconds. The
level of pain was further 15-20% of the initial VAS. In addition, the pain
passed localized - concentrated in the upper jaw. Such pain intensity remained
about 4 hours, and then decreased pain. [5] Xenon - immunocorrector
known, it has a mild immunostimulation effect,
anti-inflammatory effect, which will undoubtedly have a positive effect on the
postoperative healing in dental implantation.
Pain sensitivity of tissues
in the area of the proposed intervention implantalogic
characterized by significant differences between patients: pain threshold is
different in 19.3 times, and the endurance level of pain in 20.5 times in the
studied group of patients [6]. According to clinical data after dental implant
pain intensity on a scale of VAS after inhalation of oxygen-xenon mixture
decreased to 46,8 ± 9,7 [7]. The
available scientific evidence suggests that xenon can influence the formation
of adaptation on tolerance type and can lead to a decrease in the activity of
stress-realizing system. Xenon has an antistress effect through reversible
blockade of synaptic transmission mediators of excitatory cells (MDA-receptors)
through potentiation of effects inhibitory neurotransmitters (glycine, GABA)
through blockade releasing hormone stress implement body systems (adrenalin,
glucocorticoids), through the change of blood flow [8]. Therefore, the use of
xenon therapy has a positive impact on the human body systems to adapt [9].
Thus, the analysis of data
in the literature devoted to the study of oxygen-xenon mixtures, allows us to
establish that the main advantages that characterize the innovative technology
is a significant reduction in emotional stress, rapid onset of analgesia, a
strong analgesic and anesthetic effect without loss of consciousness, lack of
toxicity, teratogenicity, mutagenicity
, biotransformation. Conducting innovative anesthesia is characterized by easy
handling, as well as improves the effectiveness of treatment and to avoid a
whole range of complications and side-effects of pharmacological agents. No
wonder the noble gas Xenon anesthetic recognized third millennium. These
qualities are necessary for a peaceful and relaxed state of the patient, which
in turn will allow the dentist - surgeon to focus solely on the technical part
of the operation, as well as create favorable conditions for the best possible
postoperative wound healing and healing of the implant.
