 GNATODINAMOMETRIC STUDY AFTER BONE GRAFTING IN COMPLEX REHABILITATION OF PATIENTS WITH CONGENITAL CLEFT OF THE UPPER LIP, ALVEOLAR PROCESS AND PALATE

 

Relevance: The congenital cleft of lip, alveolar process, hard and soft palate (CCLAPP) is serious developmental abnormality of dental alveolar system which is characterized by the expressed structural and functional disturbances. After the main stages of complex after treatment of patients with a congenital cleft of lip and a palate there is a defect of an alveolar process of the upper jaw elimination which is impossible without carrying out a bone grafting.

Materials and methods: From 2009 for 2011 in the department of pediatric maxillofacial surgery of 3-rd clinics of TMA examination and surgical treatment of defect of an alveolar process of the upper jaw at 15 patients aged from 8 to 18 was conducted. The bone grafting of cleft alveolar process of the upper jaw with use of spongiform substance from an ileac bone and umbilical cord was done.

For the purpose of an assessment of functional efficiency of basic teeth conducted gnatodinamometric researches. Gnatodinamometric measuring was investigated by us on incisors and canines, i.e. on teeth being on the edge of a ñleft. To all patients it was carried out an bone grafting of defect of an alveolar process of the upper jaw with autogenous bone graft and umbilical cord.

The received results and their discussion: At patients about one - and bilateral congenital cleft lip and a palate after cheylo and palatoplasty were taped teeth which biomechanical indicators are considerably lowered, is inadequately perceived normal chewing loads, there is a mobility of tooth that can bring to its the subsequent loss.

After carrying out bone grafting of defect of an alveolar process of the upper jaw with autogenous bone graft and umbilical cord use we received augmentation of a gnatodinamometric indicators at incisors with 5,11±0,66 to 8,34±1,25 kg, at canines - with 7,4±0,82 to 12,1±1,8 kg.

Conclusions: The received results show expediency of carrying out an bone grafting of defect of an alveolar process of the upper jaw with use of an ileac bone and a cord at (CCLAPP) allows to rehabilitate patients of this category functional anatomically completely.
