The influence of dental implants installation on indexes of  Schiller-Pisarev test.

 

     Last years the implantology precipitously progresses. The achievement of expected results leaded to new  issues emergence about used materials.

     Although the orthopedic treatment is very important, should take into account that during the treatment foreign bodies entered into the oral cavity and located there for long time, these foreign bodies from the materials that do not suited to the human body.

     Ecspecially, it should be noted   that the reactive state of the organism is important:chronic diseases, the work and mode of life conditions ,mobility of the nerve system , allergization, immune system condition.

    Established by clinical observation, that the pathological changes in the oral cavity arise during the prosthesis by  dentures made from  different alloys . Manifestations of  metal sensibility can be different. There is a definite link between metal dental prostheses and various pathological condition of mucuos membrane.

     The object of our investigation – monitoring of indexes of Schiller-Pisarev test in orthopedic patients after  installation the  nonremovable implants, made from different materials, on titanium implants.

    Material and methods

    For our investigation we examinated 86 patients(57 male and 31 female) in age from 21 to 40 years, which were carried out orthopedic treatment with using the interosseous titanium implants. For examination formed  2 groups: 1 group – patients used mataloceramic dental prostheses,and second –

patients used non-metal ceramic prostheses. In each group the part of patient s had therapy by preparation Erbisol 1 ml intramuscularly  everyday by course for 10 days after nonremovable implants installation  – this was subgroup B , other patients- subgroup A. Erbisol – has properties of adaptogen, immunomodulator, that can activate reparation and regeneration.

      Schiller and Pisarev test based on detection the glycogen in gingiva, the glycogen content increase during inflammation due to violation of  epithelium keratinization. In healthy gingiva there is no glycogen or there are traces of glycogen.

     In our study for determination of inflammatory process we used mathematicized Schiller and Pisarev  test.

   Results and discussion

      During the first month after prosthetics the changes had statistically insignificant character in both groups of observation.

      By 2 month noted  progressive increase of indexes in both groups, that we associated with reaction of the body to conducted prosthetics : in 1 subgroup the indexes were 1,69±0,034 points, that was 26,1% more than before prosthetics (p<0,01), in subgroup B(with using Erbisol) – 1,59±0,041 points, that was 16,9 %  higher than indexes of Schiller and Pisarev test before beginig of prosthetics (p<0,05); at the same time the indexes in second group were : in subgroup A- 1,66±0,042 points,and in subgroup  B 1,68±0,041 points(p<0,05).

    By 3 month of investigation  the indexes were different from source indexes:specially tne ondexes increased in 1 group ,in this way in subgroup A – 1,87±0,042 points,that 39,5% higher than indexes before implants fixation (p<0,001), in subgroup B – 1,21±0,047 points, that 25,7 higher that the indexes before prosthetics . Another picture was in 2 group :in subgroup A – the indexes stayed quite high, but there was tendency to  stunting,and in subgroup B – the indexes were 1,55±0,045 points(p<0,05)- statistically insignificant character.

    In next term of observation (5-6 month) the indexes of Schiller and Pisarev test in orthopedic patients of both groups  approached to indexes before prosthetics and had statistically insignificant character (p>0,05).

Conclusions

    As a result  of conducted monitoring of indexes of Schiller-Pisarev test in

  orthopedic patients after installation the non-removable prostheses, made from different materials, on titanium implants, it can be concluded, that the carried out orthopedic treatment causes the increase of these indexes, regardless of the material, that we relating with reaction of the body to the prosthetics.

    The conducted observations showed the efficiency  of used preparation Erbisol in possible complicantions prophylaxis in the short and long terms, that leads to significant reducing the time of adaptation after prosthetics. The use the immunomodulator Erbisol allow to reduce drug load on patients body at the expense of it unique features. During the observation we have not identified any negative processes or side effects , assosiated with receiving the Erbisol , moreover patients noted the improvement of their condition, increase  in performance.
