"Impact of prosthesis fixed
structures on the biochemical
parameters of mixed
saliva"
At present, it is
safe to say that the method of
dental implantation has taken its
rightful place among other dental
procedures. In most cases, the
implants are made from titanium. With the growth of allergic reactions to various metals and metal alloys used in dentistry,
titanium regarded as a crucial alternative. Thanks to the
wonderful and incredible stability,
biocompatibility of titanium, this metal has attracted the attention of
orthopedics. . Due to that it does not corrode
and does not give the free metal ions which are capable of surrounding the implant or prosthesis to cause
pathological processes. But, in
spite of the unique qualities, we cannot
exclude the development of
adverse electrochemical processes using dental alloys in
fixed prosthesis with reliance on titanium implants. Implantation
is often accompanied by various
complications of the soft tissue
and bone, these effects lead to inflammatory and
atrophic processes, in which the leading role played by the materials of construction
of prostheses and implants.
Priority and
promising area of medicine is the development of new technologies increases the functional reserves of a healthy person, it is important for the rehabilitation of patients with surgical interventions. The aim of our
study is to determine the level of
acid-stable inhibitors of mixed saliva orthopedic patients as a criterion for assessing the compatibility
of implant materials and various types of fixed prostheses, prosthetic
adaptation processes in various types of fixed restorations to implant.
A change in their balance sheets is a typical manifestation
of the body's response to injury
at the molecular, cellular and systemic
levels.
To determine the level of acid-stable inhibitors
(CSIs) in saliva sample was pretreated for depositing acid-labile proteins. For
this 1 ml of saliva was mixed with 1 ml of 0.05 M Na-acetate buffer (pH 1.4)
and heated in a water bath at 60 º C for 20 minutes. To determine the CSI in
temperature-controlled spectrophotometer cuvettes prepared two samples -
experimental and control. Experimental sample contained 1.4 mL of Tris-HCl
buffer (pH -0,08). 0.5 ml of saliva, and 0.1 ml trypsin (10 mg) in 1 mM BAEE
solution was stirred rapidly and increase the optical density measured at 253
nm against the sample by spontaneous hydrolysis of the substrate. Counts were
doing every minute for 4 -5 minutes from the linear portion of the curve
depending on the optical density of growth for 1 minute for the test and
control samples. The difference between these values is used to
calculate the CSI in IU / ml. To conduct
the study were examined 38 people (22 women and 16 men) aged 21-40 years. By
selecting patients, we take into account the overall health, past illnesses
anatomical and physiological features of the oral cavity. Patients were divided
into 2 groups: Group 1 included 20 patients who, after implantation were fixed
metal-ceramic prostheses based on cobalt chrome alloy. The second group - 18
people have been attributed to patients who are in prosthetics were made
without Metallic ceramic prostheses implant. Each group was allocated to 2
subgroups: subgroup A - without any additional therapeutic interventions,
subgroup B - patients after fixation of permanent structures in the daily \ m
into the gluteal muscle was administered the drug "Erbisol" 1 ml
course of 10 days. Also examined 15 healthy individuals not suffering from
dental pathology - a control group of Orthopedics treatment was carried out by
two staged procedure endosseous implant screw implants "Uimpl" with
further prosthetics different fixed structures. In conducting the study, we
used the drug "Erbisol" - an immunomodulation drug and adaptive.
Erbisol class drugs affect only the unbalanced system, damaged organs and
tissues and are almost indifferent to the healthy organism without causing
adverse reactions. Processing of the results of research performed with the use
of standard software package Microsoft Office 2000 on a PC-class Pentium.
Analysis of the indicators
of acid-inhibitors (CSI)
of the mixed saliva of orthopedic
patients with different types of fixed prosthesis implant-supported by 1 month of
observations in all groups showed
a statistically non-significant observation
changes. In the following 2 months of observation there is a progressive increase in the CSI in both groups.
In the subsequent period
of observation (5-6months) indicators
of acid-stable inhibitors of mixed saliva orthopedic patients in the two groups of observations approaching targets, acquiring
statistically insignificant nature in relation to the control.
The study indicators
of CSI mixed saliva orthopedic patients with different types of
fixed prosthesis implant-supported
been an increase performance regardless of the type of prosthesis to 3 months
after the prosthesis. The fastest
recovery indicators CSI observed in Group 2,
where the used metal-free ceramic prosthesis -
to 4 months of
observation. The use of an
immunomodulation and adaptogen
"Erbisol" contributed to a
more rapid normalization of the
level of CSI: in group 1, which used metal-ceramic
prostheses - to
5 months, in group 2 were used where metal-free
ceramic prosthesis - to 3 months.
