Features of the organization of drug supply of the Republic of Kazakhstan

 

          The right to health in the Republic of Kazakhstan (RK) is fixed by the Constitution and implies the equal rights of citizens to safe, effective and quality health care (HC) [1]. Drug supply (DS) - one of the most important components of the MP and no small measure of social support, directly contributing to improving the health and quality of life [2]. The main mission of the pharmaceutical market in Kazakhstan is to provide the population with quality and affordable medicines (drugs) [3].

Existed in the late 90s in Kazakhstan rigidly centralized system of public provision of public drug was completely destroyed, and formed the organizational and economic relations in the sphere of drug trafficking were not able to ensure the implementation of one of the fundamental human rights guaranteed by international law and the Constitution of Kazakhstan - the right health. The ongoing reform of the EA population and health care institutions are not allowed to solve the problem of local drug production and availability of imported pharmaceuticals, resulting in approximately 75% of the citizens of Kazakhstan did not receive adequate medical therapy, and the sphere of drug trafficking are increasingly attracted the attention of criminal organizations, especially in terms of fraud drugs that counter to the objectives of social and economic security [4].

In Kazakhstan, the question has risen most sharply since the beginning of the 90s of XX century, when both appeared unfavorable trends in terms of public health and in our market appeared counterfeit drugs. Against the backdrop of unfavorable trends in terms of public health criminalization of trafficking in drugs became more and more threatening. At present, in the form of DS RK is the formulary system, which is based on drug selection with the best combination of "security applications - the effectiveness of the treatment - the cost of treatment," and they are being rational use of the health system.

The state is now an active player in the Kazakhstan pharmaceutical market, but not only in terms of legal regulation, but also as sofinansist drug consumption. In order to effectively manage inventory, quality LO population, the Government of the Republic of Kazakhstan, as part of the National Fund of the welfare of the people "Samruk-Kazyna" in 2009 created the "SK-Pharmacy", designed to address the major challenges LO population in the framework of the guaranteed health care (GHC) to the population as a single distributor (ED). In accordance with the objectives set out in the Charter of the "SK-Pharmacy", ED manages inventory, including - Purchase, storage, distribution and supply of quality drugs and medical products (medical devices) as part of free health care [5].

Used in the " SK -Pharmacy" methodological and logistical tools to manage inventory to provide state-guaranteed MRE programs provide equity and increase the access of the population to the most important drugs [6].

In this case, the most important factors that influence the above processes are the following processes: the definition of a real need for outpatient drugs and inpatient MO-based accounting change targets of various diseases and populations of certain categories of citizens in need of subsidized drug coverage; commissioning brand new high-tech medical institutions using international standards of pharmaceutical care; underdeveloped domestic pharmaceutical industry and others [7].

The domestic pharmaceutical production is generink, the market demand for innovative drugs is met through imports (up 70% in value terms). Increasing competition from foreign manufacturers significantly complicates the process of organization and sales management in enterprises, obtaining competitive advantage made possible with the concentration of domestic producers with other participants of the pharmaceutical market, through vertical and horizontal integration in a large holding structure [8].

The main specific features of today's pharmaceutical market in the Republic of Kazakhstan, the presence of centralized drug procurement budget: 40% traded on the pharmaceutical drug market (including the full cost of hospital segment) to be paid from the budget.

In the current model of inventory management using the core model of the interaction Consumer - Supplier. When using the model, which is the central figure of the consumer data is delivered directly to the consumer, which greatly facilitates the task of getting the information it needs.

Markets of developing countries, including Kazakhstan, are subject to economic, political, social, legal and legislative risks, different risks than more developed markets. The current system of the Republic of Kazakhstan to the centralization of LO when filling drug programs through SBP Single distributor "SK-Pharmacy", allows for more efficient use of health care resources and social protection of the poor.

Thus, in the organization of the RK LO underwent changes, improved, and operating circuit pharmaceutical supply of the population has effective controls inventory. Feature of the organization is to centralize LO RK filling in drug programs through free health care single distributor "SK-Pharmacy», which allows for more efficient use of health care resources.

         This paper presents an overview of the dynamic changes in drug coverage citizens of the Republic of Kazakhstan, taking place in recent decades. It shows the current system and its basic principles. In this case demonstrated that the centralization of filling in drug programs through SBP Single distributor "SK-Pharmacy" allows for more efficient use of health care resources.
