On assessment
of the quality of drug supply
in the Republic
of Kazakhstan
Drug provision of the population of the Republic of Kazakhstan is an
integral part of the health care system and predetermined by the need to
preserve and promote the health of the population [1,2]. The criteria for
evaluation of the pharmaceutical market as the market any goods are concepts
such as capacity, structure, pricing policy, legislation on a particular
product, the interaction of market participants and others [3,4]. Along with
this, it should be noted that the population as a set of buyers, in general,
characterized by the size of the effective demand and consumer preferences, and
also specifies the actual size and structure of segments of the pharmaceutical
market [4]. Over the last 3 years of public drug coverage programs provide free
medical care in Kazakhstan through a single distributor
"SK-Pharmacy". One of the components of a comprehensive evaluation of
the quality of the current drug coverage is public opinion, which, in our
opinion, is essential to the future development of the pharmaceutical market.
Purpose - to examine public opinion about the quality of drug supply.
The ratio of the main subjects of health care to the
centralized drug supply program of free medical care was studied by
interviewing patients, physicians, outpatient and hospital organizations, and
pharmaceutical institutions of Astana.
We conducted a survey of 42 pharmacists of 20 retail pharmacies medicines.
It was important to find out the attitude of employees of pharmacies to changes
in the availability of certain categories of the population to receive free
drugs under the reimbursement program. As studies have shown, the vast majority
of pharmacists were women. Their composition was 78.5% of women aged under 39
years. Meanwhile, 65% of respondents had been working for 1 to 5 years, 16.6% -
more than 5 years, and 18.3% of the respondents it was limited to one year.
Study respondents' opinions about sales volume of drugs in
recent years has shown that 40% of respondents believe that during 2011-2012,
it has increased, none of the respondents did not consider that sales have
dropped significantly. The results of the survey respondents who usually buys
drugs showed that mostly buying drugs elderly (68.5%).
Analysis of the views of pharmacy workers on what to buy more drugs at the
pharmacy, where they work, indicates that the population often prefer to buy
generic drugs - 53.4%, at least - brand-name drugs (18.3%) and homeopathic
remedies (13.3%).
The study of security pharmacies drugs issued preferential categories of
the population revealed that they regularly throughout the year can provide
only 24% of the studied pharmacies. It is established that at the beginning of
the year, especially in January and February, the absolute number of the
surveyed pharmacies can not fully provide the population with subsidized
medicines, as well as 72.0% of pharmacies - at the end of each month. And, more
often (64.0%) pharmacies are deficient expensive medications, and the deficit
is observed in the range of 32.0% studied pharmacy. The
leading factors affecting
the timeliness and
completeness of
availability of subsidized
medicines are:
the lack of the necessary continuity between clinics and
pharmacies (16.0%)
and lack of awareness of
the population in drug
benefit (40.0%).
In order to explore satisfaction with the drug supply in the preparation of
free hospital care was conducted a survey of 106 patients receiving inpatient
care and 64 physicians working in hospital organizations Astana. It was also
interviewed 42 mothers of children who are hospitalized at children's
hospitals, 16 pregnancy, labor and postpartum women, as well as 68 adult
patients admitted to somatic hospitals.
The results of the analysis of responses to the question "Did you know
that in recent years drug supply kinds of free hospital care has been
centralized service in the face of" SK-Pharmaceuticals'? "Showed that
more than half of the respondents who use hospital care, do not know about the
new order of drug providing the country. Of the total number of respondents,
91.3% were satisfied with the completeness and quality of drug supply, and 8.7%
- no. The most common reason for dissatisfaction was the lack of essential
drugs and the high cost of acquisition (52.4%), 19.0% cited a lack of
acceptable forms, dosages, liquid formulations for children, etc.
Studies have shown that the level and completeness of availability of
discounted drugs is largely dependent on the quality of the bid, the
effectiveness of outpatient organization, and to a lesser - the quality of
pharmacies. It is obvious that the "IC-Pharmacy" should enhance
efforts to identify the most complete needs of people with drug benefits,
improve the efficiency of interaction and feedback with regional health
authorities. It is shown that the best knowledge in a particular
category of patients benefits provided by law, is central, activating all the
stages of this socially significant process. It is, first of all, for the
expansion of the list of beneficiaries of dispensary groups of patients with
socially significant diseases, cancer pathology, rare diseases, which require
the use of expensive drugs. It is obvious that the priority in this work should
be priority to ensure preferential drugs in young children, pregnant women,
people with disabilities, people with diabetes, tuberculosis, mental disorders,
diseases of blood-forming organs.
Thus, our case studies show that the centralization of drug content of
programs of public provision of free medical care will improve the quantitative
and qualitative indicators that characterize the obligations of the state in
ensuring the constitutional rights to the full drug coverage. It is important
to note that more and better drug coverage of the population by the
concentration of powers and responsibilities in the hands of a single
distributor of - "SK-Pharmacy", it was a timely step requires its own
institutional development and future use of socially significant technology and
cost-effective mechanisms.
