Automated information-analytical systems in health care

 

The overarching objective of any country is the health of its citizens, which is able to maintain a well-functioning health system.

Currently in Ukraine held a number of health reforms in the pilot regions, which include Vinnitsa, Dnepropetrovsk, Donetsk oblast and Kiev.

Some steps in this direction were made successfully. Within the project "E-region" on all "emergency" machines were set GPS-system. There is creating a single regional center dispatching. This allowed not only reduce "emergency" travel time but also get a 30% reduction in fuel consumption.

The regions have launched a pilot project on telemedicine. This allows physicians to get remotely video tutorials of medical experts.

But despite the active state work, there is still not enough funding for complete medicine modernization. Moreover, there is a need to improve the quality of medical care at the primary level, as from assisting in primary health care facilities depend on the further route of the patient , so as the further patient route as well as the speed of his recovery is depended on the assist in primary health care facilities.

Quality and efficient management of economic, financial, statistical activities of medical institutions leads to the improvement of medical orders system by keeping statistics on the detection and prevalence of various diseases, provides a timely opportunity to respond to the people needs.

Moreover, the increase in the rational use of medical resources contributes to providing better health care, which in turn increases the overall health of the population.

Workforce with a high health status is essential for the competitiveness and productivity of the country. People in poor health may not realize their potential and work less efficiently. Investments in health are extremely important in both economic and moral aspects.

Radical changes in the field of Health in Ukraine must precede certain information and technological changes. One of the most important areas for the provision of health services is to provide information and analytical support as medical staff and directly patients.

Effective functioning of health depends on the quality and timeliness of management decision-making to improve the health status of the population, improve the level of care and the identification of perspective directions of the industry development. Flexibility and accuracy increasing of management decisions is only possible with effective information and analytical tools that can quickly respond to the socio-medical needs of citizens.

Object of study (design) is a medical process automation, optimization of medical personnel work and improvement of medical procurement system for the purpose to improve the socio-demographic indicators of Ukraine.

Solved problem is connected with the creation and economic feasibility of an improved system of health care provision to population.

This problem is of great scientific and practical interest in the development of automated information-analytical system, which aims to improve the medical institutions effectiveness in Ukraine , to improve the quality of medical services and rationalization of conducting medical processes.

The developing of an automated information-analytical system "E-hospital" for medical institutions is necessary to establish processes of care to the population of Ukraine. That provides the phased implementation of an automated system for hospitals and other health institutions in the country.

Development and implementation of information-analytical systems of automatization medical processes is an urgent task. Thanks to the implementation of such systems will be possible:

-                   the optimization the of the medical staff operation;

-                   reducing errors in the treatment;

-                   the maximum simplification of medical records from a single electronic archive;

-                   will be possible to obtain real-time access to patient information that definitely will allow medical personnel to maintain mobility and to make better medical decisions;

-                   improving the effectiveness of functioning of the medical institutions;

-                   improving the productivity of health care personnel;

-                   improving the delivery of health services and medical process that is always relevant.

Since Ukraine has free medical care, that is financed from the state and local budgets, hospital budget is always limited and rational use of medical resources is critical, so relevant is the use of the system, which will help reduce costs and increase revenue by reducing inefficient use cases resources, and their more rational use. This is achieved by analyzing and forecasting using the information-analytical systems.

It should be noted that such systems are scalable and extensible.

Optimization of the medical personnel work. Reducing number of technical errors in treatment. Making more weighted medical decisions. Reducing the cost of inefficient resources use. Improving the health services delivery system and medical process. Concentration of doctors in the treatment process, and not on the implementation of formal procedures. Improving the efficiency of health services delivery system. Creating methods and guidelines for the development of an automated system in a medical facility.

Based on preliminary estimates predicted that the use of this system will reduce the conducting of medical documentation to 90%, clearance of the patient in the emergency department will be reduced to 70%. Time spent on medical records by physician, is approximately two hours a day, by a nurse – one hour. Implementation of "E-Hospital" in hospitals will significantly increase the quality of medical service.

Use of automated information-analytical system "Electronic Hospital" will allow to change the concept of medical records, to use a rational approach in the procurement of medicines and improve the process of care.

Such automated information-analytical systems can be used in medical institutions of all forms of ownership and profile. In such systems may be interested as a city, district and provincial general hospitals.

Besides substantially reducing costs of inefficient use of resources, implementation of new software will improve the performance of health care personnel and the quality of health services and improve the overall level of citizens health.

Realization of automated information-analytical systems for medical institutions requires performing some steps:

1.                 Collection and preparation of data. Setting objectives that should solve the analytical system. Processing of incoming information (results – identifications of system key performance indicators, implementation of technical specifications).

2.                 Purchase of equipment. Purchase of materials and equipment for the project: personal computers, printers. Connecting to the Internet, conducting network (results – material and technical support).

3.                 Development of software. Architecture design of the automated system "Electronic Hospital", the development of software, creation of primary documentation; saturation the database of information. Each step includes the development, analysis and testing (results – materials and equipment are prepared for trial operating of system).

4.                 Acceptance testing. Focus group training. Customer support. Correction of defects found during testing (results – product has been tested and ready for operation; testing by group from the customer (UAT – acceptance testing). Completion criterion is product approval by the group. In the case of detecting errors within a month after the application they are corrected).

5.                 Analysis of system efficiency. Experimental evaluation of system performance, increase productivity, assessment of increasing of health personnel productivity. Carrying out theoretical calculations of reducing financial costs for the rational use of medical resources (results – improving the health care delivery system).

6.                 Creating methods and guidelines. Assessment of the overall effect of using the system (results – creating methods and guidelines for the development and operation of the automated system "Electronic Hospital" in a medical facility).

7.                 Introduction. Summing up the project. Making presentations and progress report.

8.                 Summing up the effects.

The main expected results of the modernization of medicine are:

-                   the creation of automated management of economic, financial , statistical activities of medical institutions , as well as keeping the information-analytical processing of medical data;

-                   creating an electronic archive of the patients;

-                   creating a recording program for patient receiving over the Internet, or specially installed terminals in institutions for the provision of socially important services to the public;

-                   improving of management institution, improving medical orders system by keeping statistics on the detection and prevalence of various diseases;

-                   improving the rational use of medical resources;

-                   improving the economic efficiency of medical institutions;

-                   receiving information about the patient from electronic archive of patients;

-                   reducing the time waiting in line to a doctor;

-                   allocation of more time for inspection and examination of patients;

-                   reducing the number of unsuccessful appeals outpatients in an institution;

-                   improving the quality indicators of health status.

Besides substantially costs reducing of inefficient use of resources, implementation of this system will improve the performance of health care personnel and the quality of health services, improve the overall level of Ukrainian citizens health.
