ABOUT DYNAMIC AND STRUCTURAL CHANGES IN INCIDENCE

 OF CIVIL SERVANTS

It is known that indicators of the general and primary incidence, are the most important indicators of quality of health of the population and efficiency of the rendered case historical-polyclinic help [1, 2, 3].

Proceeding from this parcel, dynamics and structure of incidence of the attached civil servants in 2007-2012 was analyzed and estimated.

As showed the conducted researches, in analyzed year’s age-specific structure of civil servants, as a whole, didn’t undergo to considerable changes.

However, the detailed analysis of their age-specific structure testifies that in 2009 the share of persons at the age of 76-90 years was smaller, than in 2007 (42,4% against 39,3%, respectively), and in 2012, in comparison with previous 2007 and 2009, even more decreased and made 38,4%. The share of elderly people at the age of 90 years also is more senior significantly didn't change and hesitated ranging from 4,0%-4,1%-3,8%. Thus the share of persons till 63 years increased and made: 27,0%-in 2007, 33,0%-in 2009 and 38,4% - in 2012.

Thus, it is established that in hospital the regressive type of the attached contingent that causes high complexity and intensity of medical care remains.

Explorations showed that in 2012 in comparison with 2007, among acting civil servants the essential increase in number of visits in a year (growth + 26,0%) and coverage of KPMO (height of +2,3%) and, the decrease which has followed it of the general (-15,4%) and primary (-49,9%) incidences is registered. Among the former civil servants the average of visits in a year in 2012 increased by 3,6% in comparison with 2007; coverage of KPMO was former, but, nevertheless, indicators of the general and primary incidence decreased: respectively for-7,9% and-37,7%.

It is shown that in all analyzed years in structure of the general incidence of the former civil servants, the first ranked place belonged to diseases of bodies of blood circulation with small fluctuations, both in absolute, and in relative values; the second ranked place belonged to diseases of GIT (gastrointestinal tract) and the third - to diseases of genito-urinary system.

The analysis of structure of primary incidence of the former civil servants showed that 1 ranked place here belonged to diseases of respiratory organs.  The second place in primary incidence belonged to the traumas, which number in 2007-2012 decreased by 6, 7%.

In 2007 the third ranked place in structure of primary incidence of the former civil servants was taken by diseases of digestive organs (11, 8%), and in 2009 and 2012 decrease in this indicator (respectively, for 6, 2% and 31, 0%) was observed.

When studying structure of the general incidence of acting civil servants it was revealed that here, during all analyzed periods, unlike former, on 1 ranked place there were diseases of digestive organs; on the second - diseases of genito-urinary system and on the third - an illness of respiratory organs. And in 2009 in comparison with 2007, growth of studied indicators (respectively was noted: +28, 5%; 13, 3%; 28, 3%), and in 2012 - their decrease - for 5, 0%; 10, 2% and 9, 2%.

It is established further that in all studied years, the structure of primary incidence of acting civil servants differed from that among the former. And 1 ranked place, as well as among both groups of the attached contingent, diseases of bodies of blood circulation borrowed, on the 2nd ranked place there were diseases of respiratory organs, on the third - an illness of genito-urinary system. In 2009 growth of studied indicators, respectively is revealed: for 33,3%; 13,6% and 12,0%; and in 2012 – their relative values decreased, respectively on: 27,1%; 52,3% and 44,2%.

Thus, the conducted researches showed that dynamic changes in level and structure of the general and primary incidence of civil servants are important for definition of the main priorities of preventive, medical and diagnostic and medico-rehabilitation activity. Thus the main efforts in the organizations of medical care have to be directed to the former civil servants on improvement of quality of dynamic supervision, efficiency of the out-patient and polyclinic and stationary help, and among acting civil servants - early identification of the prepainful conditions and carrying out timely improving actions.

Annotation

In work the analytical material of dynamics and structure of incidence of the attached civil servants in 2007-2012gg is presented. It is established that in hospital the regressive type of the attached contingent that causes high complexity and intensity of medical care remains; in structure of the general incidence of the former civil servants 1 ranked place belonged to diseases of bodies of blood circulation; To 2 diseases of HSSH and 3 diseases of genito-urinary system. It is shown also that dynamic changes in level and structure of the general and primary incidence of civil servants are important for definition of the main priorities of preventive, medical and diagnostic and medico-rehabilitation activity.
